ANALYSIS 5S IMPLEMENTATION OF MANAGEMENT SYSTEM USING FAULT TREE ANALYSIS (FTA) METHOD

Abstract

Along with the times create any agency or company to compete with each other to improve the safety and comfort systems in the workplace. Therefore, this study aimed to analyze the implementation of 5S management system in hospital X that performed the analysis with fishbone diagram method of each factor 5S. 5S factors that seiri, Seiton, Seiso, Shitsuke, and Seiketsu. Further calculation by using Likert comparing the first factor with the total overall factor to get a major factor. So we get the Seiketsu factor as the main factors of the implementation of a more mainstream conducted at Hospital X. Further advanced analysis using the FTA to obtain cause - the main cause of Seiketsu and the results obtained in the form of recommendations on the implementation of 5S management system at Hospital X especially Seiketsu factors as well as recommendations as a reminder in the form of 5S posters as well as a form of controlling the implementation of 5S. Therefore, the expected improvement recommendations can be implemented by the suggestion to Hospital X.
