Recent and Future Trends in Population Health Management

Abstract

Population health depicts a gigantic move in the social establishment of human services. It represents the medical outcomes of different clusters of individuals. These clusters may be defined by multiple demographic and environmental factors. One of the major objectives of population health, in the management perspective, is to completely diminish the diversity in medical fitness or at least deduct the differences substantially. The medical situations within groups of individuals need to be distributed evenly, at least within similar groups. The use of populace well being depends intensely on present day innovation, mostly, information/investigation and mobile well being solutions. Anticipating a drastic shift from traditional health care for people, this paper aims to put forth a detailed description of Population Health Management (PHM), the objectives and benefits of PHM and the emerging trends in PHM.
